ODP Update: Registering Users for the Provider Staff COVID-19 Tracking Form

The Provider Staff COVID-19 Tracking Form became effective June 10 as a web-based tool for providers to report and track suspected and confirmed cases of COVID-19 amongst provider staff. The ODP online survey for providers to register primary and secondary users allowing access to the Tracking Form is closed.

If your organization did not yet register primary and secondary users, it is important that you contact Health Risk Screening (HRS) Inc. The Provider Staff COVID-19 Tracking Form is available here.
