Supports Coordinator

Life Sharing is a service that has one of the highest rates of satisfaction aligning with an everyday life (Independent Monitoring for Quality (IM4Q)). The Office of Developmental Programs (ODP) wants this service to continue to grow and is providing information to educate everyone about the Life Sharing service. The first step was to create promotional materials to assist with their goal. These documents were created with the assistance of Providers of Life Sharing, Administrative Entities, ODP, and feedback from Supports Coordinators.

In the near future, webinars will also be provided on Life Sharing, and ODP will communicate with stakeholders when these dates are set.

What are These Documents?

There are two documents available:

  1. The Life Sharing Brochure is intended for individuals and families with the hope that Residential Providers that offer the Life Sharing service and Supports Coordination Organizations will print these out and disseminate at their headquarters as well as pass them out as appropriate. This document easily describes what Life Sharing is at a high level.
  2. A one-page document, “Information to Help Supports Coordinators Talk to Individuals and Families About Life Sharing,” is intended for Supports Coordinators to use when having conversations with individuals and families. ODP hopes Supports Coordinators will utilize this form at Individual Support Plan (ISP) meetings and on a more routine basis to help individuals and families understand what the Life Sharing service is and to help them understand there are multiple options for residential services.

The Office of Developmental Programs (ODP) has shared ODPANNN 25-091. This communication announces the release of an updated training course, “Introduction to Conflict Resolution for New Supports Coordinators,” a 1 credit hour course that is part of the SC First Year Training requirements. This training builds on the concepts discussed in the “Introduction to Facilitation” webcast and provides a framework for working through disagreements within the team and how to resolve them. Before completing this training, completion of the Introduction to Facilitation for New SCs training is recommended.

Introduction to Conflict Resolution for New Supports Coordinators can be found on MyODP as part of the SC First Year Trainings.

The Office of Development Programs (ODP) and its Independent Monitoring for Quality (IM4Q) technical advisors, in collaboration with the IM4Q Statewide Steering Committee, have begun partnering with Support Coordination Organization (SCO) representatives to enhance SCs’ understanding of IM4Q and their role in the process. The IM4Q SC Tip Sheet outlines the IM4Q process, SC responsibilities, how to inform the individuals they support, and what occurs after the interview. SCOs are encouraged to share the tip sheet as a reference when training new SCs and with experienced SCs as a refresher.

Please review ODPANN 25-051 for further details and information.

For an individual to remain healthy, safe, and satisfied, recognizing, mitigating, and responding to risks are the responsibility of the entire team. Supports Coordinators (SC) have a critical role and responsibility on the team to monitor all the services that are being provided to the individuals they support. Because providers of Residential Services have a unique responsibility to protect and promote the health, safety, and rights of individuals they support in all aspects of their life, 24 hours a day, 7 days a week, this training will specifically focus on an SC’s authority and responsibilities when monitoring Residential Services.

This is a 4-part training that is provided for and recommended by the Office of Developmental Programs (ODP) for new SCs during their first year of employment (days 61-365).

The training can be accessed at MyODP under Supports Coordinator (SCs) Monitoring Residential Services Webcast Training.

The Centers for Medicare and Medicaid Services (CMS) require a statewide process to ensure providers are qualified to render services to waiver-funded individuals. The Provider Qualification Process described in announcement ODPANN 25-014 outlines the steps the Assigned Administrative Entity (AE) and provider must follow to meet these requirements and the steps Supports Coordinators (SC) take to transition individuals if needed. This communication does not describe the qualification process for SC organizations.

Please view the announcement for additional information and details.

The Office of Developmental Programs (ODP) has shared ODPANN 24-106 and its attachment, which provides updates and identifies future plans for SC First Year Training, including a new Landing Page on MyODP as well as topics and training that were previously required in the 2017 SC Orientation. Please view the announcement and its attachment for information and details.

The Office of Developmental Programs (ODP) wants your feedback! The anonymous Supporting Families Survey seeks input from individuals who receive support from a Supports Coordinator (SC, also known as a case manager or case worker), and family members of people who receive support through an SC. The survey helps ODP learn if people and their families have the information and support they need. Your answers will help to improve services from ODP and local partners.

Why take the survey?

  • You can let ODP know your satisfaction with the support you and your family receive from the SC.
  • You can let ODP know other things that would be helpful for you and your family.
  • ODP will be able to prepare information and supports to meet your family’s and other families’ needs.

Visit here to fill out the survey in English or in Spanish.
