Melissa Dehoff is responsible for all medical rehabilitation and brain injury service issues. Ms. Dehoff attends multiple state-level meetings to advocate on behalf of members on brain injury and rehabilitation issues and is a member of the Department of Health Traumatic Brain Injury Advisory Board.

In order to promote improved coordination between Medicare and Medicaid as the rollout of Community HealthChoices (CHC) in the Southeast portion of the state gets closer, the Office of Long-Term Living (OLTL) released a CHC Medicaid/Medicare billing fact sheet. The fact sheet includes information and answers to frequently asked questions (FAQs) regarding coverage and how billing works under CHC.

The Centers for Medicare and Medicaid Services (CMS) recently announced they will be holding a National Provider Call for inpatient rehabilitation facilities (IRFs) that will focus on the changes included in the fiscal year (FY) 2019 inpatient rehabilitation facility prospective payment system (IRF PPS) final rule. The call is scheduled for Thursday, November 15, 2018 from 1:30 pm – 3:00 pm.

As a reminder, some of the key changes contained in this final rule, which will be a part of this discussion, included:

  • Revisions to coverage criteria;
  • Removal of the Functional Independence Measure (FIM) and Associated Function Modifiers from the inpatient rehabilitation facility patient assessment instrument (IRF PAI); and
  • Refinements to the case-mix classification.

Prior to the call, participants are encouraged to review the Medicare Benefit Policy Manual, Chapter 1, Section 110. A question and answer session will follow the presentation; however, attendees may email questions in advance with “November 15 Call” in the subject line. These questions may be addressed during the call or used for other materials following the call.

Registration is required to participate in this call and will close by 12:00 pm on the day of the call or when it is full. RCPA encourages all of its members in the Rehabilitation Services Division to participate in this call to ask questions, share concerns, etc. with CMS.

RCPA’s Rehabilitation Services Divisions (Medical Rehabilitation Committee and Outpatient Rehabilitation Committee) will convene their first networking session in November 2018. To prepare for this session, a Survey Monkey was created to obtain member feedback on their preference for the date and topic for discussion. The Community HealthChoices (CHC) Third Thursday webinar coming up on September 20, 2018 will include updates on CHC by Kevin Hancock, Deputy Secretary for Office of Long-Term Living (OLTL). The remainder of the webinar will focus on brain injury that includes a brain injury panel discussion facilitated by members of RCPA’s Brain Injury Committee, including: Dr. Drew Nagele, Jack Poplar, and Bridget Lowery.

The Centers for Disease Control and Prevention (CDC) released a new Pediatric Mild Traumatic Brain Injury (mTBI) guideline. This guideline is based on the most comprehensive review of the science on pediatric mTBI (most commonly referred to as concussion)  diagnosis and management to date.

This guideline was published in the Journal of the American Medical Association (JAMA) Pediatrics and includes 19 sets of clinical recommendations that are applicable to health care providers who care for patients with concussions in all settings.

The guideline provides the following key practice-changing recommendations:

  1. Do not routinely image pediatric patients to diagnose.
  2. Use validated, age-appropriate symptom scales to diagnose mTBI.
  3. Assess for risk factors for prolonged recovery, including: a history of mTBI or other brain injury, severe symptom presentation immediately after the injury, and personal characteristics and family history (such as learning difficulties and family and social stressors).
  4. Provide patients with instructions on returning to activity customized to their symptoms.
  5. Counsel patients to return gradually to non-sports activities after no more than 2–3 days of rest.

Free tools were also developed by the CDC to assist health care providers use this guideline in their practice and include:

  • A checklist for health care providers on diagnosis and management;
  • Quick guides to key recommendations;
  • Patient discharge instructions;
  • Recovery tips for parents; and
  • A letter to schools to be filled in by health care providers.

The Office of Long-Term Living (OLTL) released the Service Coordinator (SC) training materials from the Community HealthChoices (CHC) Implementation for Participant-Directed Services training that was held on August 21, 2018. The presentation and training was to provide information and process updates to SCs on the implementation of CHC for participants who utilize participant-directed services in the Southeast Region, including Bucks, Chester, Delaware, Montgomery, and Philadelphia counties.

RE: Procedure Code for Cognitive Rehabilitation Therapy Services – Processing Issue

This notice is to advise Cognitive Rehabilitation Therapy Providers that after removing service procedure 97532 SE and replacing it with the new procedure code 97127 for Cognitive Rehabilitation, a processing issue occurred causing many claims to deny.

OLTL is aware of this issue and is working on a system fix.  OLTL will send a follow up communication in the very near future advising providers of the fix and when it is appropriate to rebill.  In the meantime, OLTL asks that providers refrain from billing for Cognitive Rehabilitation in the Independence and OBRA waivers.

The Centers for Medicare and Medicaid Services (CMS) has released the updated Inpatient Rehabilitation Facility Quality Reporting Program (IRF QRP) Measure Calculations and Reporting User’s Manual (Version 3.0). This version of the manual is effective on October 1, 2018. The manual provides detailed information for IRF Patient Assessment Instrument (PAI) based quality measures, including inclusion and exclusion criteria, quality measure definitions, and measure calculation specifications. All of the materials are available on the Downloads section located at the bottom of the IRF Quality Reporting Measures Information web page.
