Statewide Tobacco-Free Recovery Initiative Survey

The Pennsylvania Department of Health, Department of Human Services, and Department of Drug and Alcohol Programs shared their commitment to support the Pennsylvania Statewide Tobacco-Free Recovery Initiative (STFRI), a five-year Centers for Disease Control and Prevention (CDC) funded project to advance evidence-based tobacco interventions in the behavioral health setting. The initiative offers state-wide consultation, training, and technical assistance for treatment providers and community partners to develop tobacco-free policies and integrate a tobacco-free recovery system of care into existing behavioral health services.

The purpose of this survey is to assess organization readiness for providing tobacco-free services. Your responses will be used to help the initiative team create trainings, tools, and other resources for behavioral health sites across the state. The initiative team may follow up with your organization to discuss tobacco-free resources as they are made available.

You will not be linked to survey responses. The survey takes less than 15 minutes to complete and will close on February 28, 2021.
