The Office of Developmental Programs (ODP) has shared ODPANN 25-113. This communication includes an updated version of the Intellectual Disability/Autism (ID/A) Individual Support Plan Signature Form (DP 1032) that was released with the ODP Announcement 25-012: Updates to the Individual Support Plan (ISP) Bulletin Attachments. After the release, ODP was informed that the MCI Number and the individual’s name needed to be added to each page of the form. This form has also been translated into Spanish.

The Office of Developmental Programs (ODP) has shared ODPANN 25-029. This communication notifies Adult Autism Waiver (AAW) SCs, SC supervisors, providers, and other interested parties of the updated AAW Updated Individual Support Plan (ISP) Signature Form (DP 1032 AAW).
The updated DP 1032 AAW is required for all initial ISPs and annual review meetings and should be used effective immediately. The form is available on the MyODP website.

The Office of Development Programs (ODP) shared updates to the Individual Support Plan (ISP) Bulletin Attachments due to the waiver amendments effective January 1, 2025.
As outlined in ODP Bulletin 00-22-05, updates to Attachment 1 (the ISP Manual) and Attachment 8 (Summary of Major Changes Made to ISP Requirements or Processes) are being announced through this ODP Communication. The updated Individual Support Plan Signature Form is Attachment 3. Bulletin 00-22-05 and attachments two and four through seven remain current until updated through other ODP Communications. Attachments 1, 3, and 8 should be used effective immediately and have been uploaded as the current attachments to ODP Bulletin 00-22-05 on the DHS Bulletin web page.
